Admitting a loved one to a care facility requires gathering several documents. Here is a list to help you get it done:
- Medicare Card
- Medicaid Card
- Social Security Card
- Private and/or Long Term Care Insurance Cards
- Birth Certificate
- Photo ID, Drivers License or Passport
- Guardianship Documents
- Power of Attorney
- Advanced Directive
- 12 Months of Bank Statements- All Accounts
- Copies of Burial Policies (know amount and if they are revocable or irrevocable)
- Copies of Life Insurance Policies (know amount and who the policy is owned by)
- Social Security Benefit Award Letter
- Verification of all Gross Income (check stubbs, award letters, employment statements)
- Retirement/Pension Benefit Award Letter
- Deeds/Titles to Home and/or Vehicles
- Mineral Rights
- Stocks and Bonds
- Documents on Livestock / Farm Machinery
- Stocks and Bonds

- Copies of any other Assets and their value
